Lessons Identified

  • The work party was very much focused on stabbing the wireline lubricator onto the BOP’s and the man riding aspects of the wireline operation. They did not identify the hazard or potential risk due to the excess chain loop from the hoist becoming snagged which ultimately resulted in the retaining pin being broken.
  • Need to maintain a focus on overall operations and not just on the specific aspects deemed high risk.
  • Need to continually review & challenge existing work practices & risk assessment to ensure they remain fit for purpose.

Incident Recommendations

Install Original Equipment Manufacturers’ chain basket under the Red Rooster hoist to collect and retain the excess chain when not required. (N.B. The risks associated with having a basket under the hoist will also need to be considered.)Update existing risk assessments for Air Hoist Operations to cover the dropped object hazard and risks associated with the excess hoist chain loop becoming snagged.
