Lessons Identified

• The spacers were not identified as potential dropped objects during the initial installation of the fingerboard

• Alternative rigging methods were available that would have prevented personnel occupying the drop zone as the fingerboard was released from it’s fixing

• The control measure agreed to retain the spacers did not work (length and grade of rope used were insufficient)

• Person allowed to work in the drop zone without any overhead drops protection

Incident Recommendations

• Ensure that an appropriate team is assembled, containing a range of disciplines and experience, to provide input into safe systems of work

• Drop zones must be kept clear of all personnel unless the hazards can be reduced to ALARP
