Safety Push Arm weighing 882lbs and it's dimensions were 12' 6" long and 8" x 8" wide.

Lessons Identified

There was no means of providing secondary retention to the SPA.The equipment was redundant tot the operation, but no decision was made to remove it from site.The SPA was not on any checklist ensuring the equipment was inspected.A visible inspection of the SPA would have failed to reveal the problem with the bolt.

Incident Recommendations

Remove redundant equipment from the workplace.Advise the manufacturer and inform them the considered reasons for the failure.Ensure that all temporary and/or permanent structures are included within the rig inspection programmes.
