Other Cause & Consequence
Hose parted (cause of hose failure not known), personnel located in close proximity to hose lift location, no mitigation in place to prevent a hose failure from impacting personnelContributing Factor
- Control of work
Description of Process
Potable water bunkering is a routine task for the platform (> 100 times per year). The 4", 70m hose, is lifted off and disconnected from a saddle and lowered to the supply vessel prior to water transfers. At rest, the hose is supported by the hooky hook and saddle and is secured to the platform to prevent movement in high winds.
Description of Incident
During the lift, when the hose end was 28m above the bunkering station, the hose parted immediately below the ferrule of the end coupling, allowing the hose to fall from this point. The hose impacted two members of the deck crew before the hose fell to sea. The persons involved reported to the medic but did not require medical treatment.
Good Practice Guidance
Put an engineered solution in place to ensure that personnel are not in danger from dropped objects during bunkering operations - ie a "hands free" operation or personnel protection.
Ensure that personnel never stand beneath a suspended load.
