The crew did not have full knowledge of the valve assembly and the necessity for completing venting pressure on pressurised lines.A false sense of security was created by the lack of pressure on the downstream vent valve on the number 3 tensionerPressure was left on the line in the event a quick change was needed to put the number 7 tensioner which was paired with the number three tensioer into serviceThe job was planned to be broken down into 3 phases. At the completion of each phase the job was to be stopped and the next phase was to be planned before going forward. The three phases did not occur.The crew were into phase 2 when the incident ocurred and there was no stoppage of work or phase planning between phase 1 and 2.
