Incident Recommendations

• Check other hoses in generators for evidence of fretting against steelwork;
• Survey generator packages with similar fuel gas manifolds for signs of stress and/or potential failure;
• Implement the use of FLIR camera when carrying out service testing of generator packages post intrustive maintenance.
