Incident caused by Human error due to lack of proper communication and high noise levels in module due to an adjacent compressor. Work permit did not identify the use of temporary hoses as a hazard, although it did mention them in the task description.

Incident Recommendations

1. Coach and raise awareness of Operation team around taking sufficient time for the job and proper communication. Also, supervisors are to set appropriate expectations.

2. Order helmet mounted communications earmuffs/micas for trial across shifts

3. Revise work control instruction to recognise temporary hoses as a potential hazard.
