Lessons Identified

There was a weep from the vent port on an electro motive valve (EMV) body. The plug was found not to be sealing correctly. The vent port provides a vent path if stem leakage occurs. It should have been piped to a safe location according to manufacturers’ guidelines when installed.

2. Plugs to be checked to ensure they are in place and are gas tight
