Lessons Identified

This failure is suspected to be due to a combination of normal work hardening of the tube end during assembly and then cracking as a result of mechanical damage which has bent the tube immediately behind the nut. The instrument tubing was not supported sufficiently along its length.

Incident Recommendations

Repair the instrument tubing and provide adequate support.
