Description of Process
Accessing an open topped basket using the fixed external and internal ladder rungs.
Description of Incident
The IP was climbing into an open topped basket using the fixed external and internal ladder rungs. Whilst swinging his right leg over the top of the basket to gain access to the inside of the basket, the IP's left foot slipped from the external ladder rung, causing the IP to lose his footing and consequently landing, straddling the basket top.
The IP reported to the medic within 1/2 hour of the incident and was medivac'd the same day.
Good Practice Guidance
It is not always good practice to use the designed external / internal ladders - in this case, access/egress ladders would have been the best means of accessing the inside of the container.
