Incident Recommendations

Blanking off the kelly bushing holes not implemented from previous incident. Risk of dropped objects from the drill floor to lower areas during rig up of wireline not fully assessed by combined crew. No suitable hole/hatch covers provided on locationfor the skid deck hatch and rotary table areas. Tarpaulins were to be used on the rig floor around the riser. Manual handling of heavy toolstrings on the rig is a two man job. Procedures not fully complied with by the combined crew requiring adequate hole covers during the operation.
