Possibility that pneumatic tools are being used without the required pressure/moisture regulator on other sitesCorrect handling of tools/equipment minimises the likelihood of damage.

Incident Recommendations

Attach copy of incident report to shipment to alert supplier.All temporary equipment is to be inspected on arrival offshore. Any deviations from operator's manual during use will be reason for quarantine.All equipment to be pre-use checked by the userAll hose connections to have whip-checks.Supervision to review equipment operator manuals for relevant issues to incorporated into job cards, risk assessment and TRIC's. Ensure that pre task risk assessment includes the potential for this kind of event and that mitigating actions are carried out prior to commencing the job.
