The JSA was inadaquate for the task.A distraction caused one of the 'lift' team to lose concentration.The IP entered the lift area for no apparent reason.

Only selected personnel to be part of the lifting process.Rigging Slinging and hoisting arrangements shall be agreed prior to the commencement of the lift.Lift Supervisors/Banksmen and Deck Foremen to ensure everyone involved in the lift understands the plan and adheres to it.
