1/ Individuals are encouraged to check that re-breathers supplied are within its next inspection due date.2/This matter was investigated by the Helicopter Company and their Contractor. Changes to procedure has been addressed to avoid a repeat occurrence.

On an agreed time after the implementation of the new procedures a full audit to be carried out by Helicopter Company to confirm robustness of there third Party suppliers system.
