Transportation of compressed gas cylinders

Description of Process

Pressurised air cylinders used for breathing apparatus were being prepared for backloading onshore.

Description of Incident

A deckcrew member was securing them by strapping them to a standard wooden pallet.

One of the bottles fell, fracturing the valve. This caused a rapid depletion of air and the cylinder was propelled around the container.

Good Practice Guidance

  • Special transporation boxes have been designed.
  • All valves were changed to a new safer type
