Incident Recommendations

Ensure that as-build drawings are correct - when the incident occurred the well drawings were assembled. The well schematic was found to be incorrect for the make up of the gas lift pipe work.Additional support should have been considered - the gas lift pipe work was isolated and a blind flange installed 3 years prior to the incident. The isolation was downstream of the spring hanger. With movement of the tree all the weight of the pipe work was absorbed by the connection from the 7" annulus.Remove redundant pipe work - the gas lift isolation could have been made at the 7" annulus outlet valve and therefore all the redundant pipe work could have been removed avoiding clutter.
