Lessons Identified

On inspection of the hose assemblies it could be seen that the point of failure was in line with an identification tag that had been welded on the swaged ferrule by the Customer. During the inspection a section of the ferrule was cut away from the hose to inspect the area under the welded tag. The following was noted.Scorch Marks were found on the hose cover directly under the two welded areas; on close inspection of these two areas the cover was found to be soft and sticky.Heat affected areas were also seen on the inner side of the ferrule

Incident Recommendations

From the investigation completed, it was found that the heat generated during the welding of the identification tags onto the swaged ferrule has affected the physical properties of the rubber. The most likely reason for the failures being the heat from the weld affecting the bond between the wire reinforcement, the inner tube and outer cover, allowing the wires to slip out from the ferrule. To prevent any re-occurrence it is highly recommended that tags are not welded on to a swaged ferrule.It is further recommended that fittings in service be checked for any welding on the swaged ferrule and if found, the connection should be treated as suspect and removed from service as a matter of priority
