One of the low pressure regulators was found to have been assembled incorrectly.

Gas had leaked back into the air intake ducting from the engine carburettors. (normal occurrence during commissioning – minimal wind speeds present).

Incident Recommendations

Rectify regulator

Inhibit gas detectors during commissioning operations;

Alert workforce to the potential of gas accumulating in the air intake ducting during commissioning;

Prevent hot work around inlet to combustion air intake
