Pass Through Tie Off Adaptor, a new item of fall restraint was being used to secure the derrickman to an anchor point. The webbing 'adaptor' is made of a webbing sling 3" wide and 2' in length, the webbing is passed through a 'D' ring where it should be glued and stitched, but this particular adaptor had not been stitched. The adaptor failed as soon as the derrickman created strain on it, the glue was not capable of the strain

Lessons Identified

The supply chain failed to identify the flaw, this being missed from the manufactirer of the item to the user of the item.
