The pipework that failed had been isolated for the main pressure test as it had not been part of the work scope. A full system pressure test should have been conducted.

• Reviewed and updated work Instructions for the reinstatement and testing of process equipment to include small bore piping and fittings
• Provide coaching to platform team on what is expected of them
• Verify Swagelok competence of individuals
• Share the learning’s from this incident with platform personnel and those planning and executing upgrade workscopes.
