Description of Incident

Whilst entering the platform chemical storage unit for materials, the chemist pushed the roller shutter door open and, as he was doing so, the shutter assembly and housing broke free from the top of the unit and dropped approximately 2m to the deck. The chemist managed to step back from the unit and avoided contact with the assembly.

Good Practice Guidance

• Ensure design is adequate (roller shutter door assembly not too heavy for the mounting assembly)

• Ensure cabinets are suitable for intended duty and environment

• Ensure cabinet and roller shutter is maintained in line with manufacturers guidance
