Lessons Identified

Emergency Response related lessons learnt:

During the evacuation process an area for improvement was identified with reference to secure communication and evacuation equipment preparedness between the Marine Coastguard Agency (MCA) and the Duty Holder.

The MCA & Duty Holder have recognised this and have subsequently changed their processes and communicated these changes to the industry.

Release of video footage taken from search and rescue helicopters may now only be approved and co-ordinated through the MCA.

Incident Recommendations

Turbine fuel and lube lines:

Duty Holders should review the integrity of all fuel oil and lube oil supply lines within turbine enclosures.

A check of all securing clips/brackets should also be made to identify the possibility of mechanical abrasion.

Fire suppression:Duty Holders should assure themselves that the design logic for these systems is robust and that firing mechanisms operate as designed.

Unit diesel emergency shut off valves:Duty Holders should assure themselves of the functionality and integrity of all emergency shut off valves (ESV) including machine specific ESV.

Review the maintenance strategy and the test regime.

With respect to trapped inventory, the Duty Holder should review the location of the ESV and its distance from the unit it is protecting.

Turbine enclosure integrity:

During the incident the turbine enclosure was not breached and as a consequence prevented potential escalation into the generation module.

Duty Holders should review the overall integrity of their turbine enclosures.
