Crew members had not identified that the crew member was working with his arm inside the mud bucket.The air supply design arrangement was inadequate.Poor communications between the IP and other crew members utilising the air supply.The crew were not suitably aware of how to correctly isolate and de-isolate the mud bucket air supply.
Incident Recommendations
Conduct and ensure that training in the safe use of such equipment is adaquate.Paint warning chevrons in the front aspect of the bucket highlighting potential pinch points.Install independant air supplies for other equipment, avoiding the need to disconnect the mud bucket air supply.Modify the controls of the mud bucket so that two valves must be operated simultaneously in order to close the mud bucket.
